VACANCY ANNOUNCEMENT Job Title: Social Worker Job Location: Harare A local faith based organization seeks the services of a social worker to support its programs. Purpose of the position The incumbent will be responsible for different case management roles with a specific focus on the provision of psychosocial support, rehabilitation assistance and family reunification.
Duties and Responsibilities
Duties and Responsibilities: The initial screening and evaluation of clients and families. Helping clients and family members deal with the many aspects of the patient’s condition – social, financial, and emotional. Acting as an advocate for clients and families – including as an advocate for the client’s health care rights. Aid and expedite decision-making on behalf of patients and their families. Educating clients on the roles of other members on their recovery team – including physicians, nurses, physical therapists, etc. Crisis intervention Providing a comprehensive psychosocial assessment of patients. Coordinating clients discharge and continuity of care following discharge. Family tracing and reunification, along with community reintegration.
Qualifications and Experience
Qualifications and Key Competencies. Bachelor of Social Work Honors Degree. At least 2 years working experience in a social work case management context. Up to date registration with Council of Social Workers Zimbabwe. Good communication skills.
