Job Description: Nurse Case Manager (Sales-Trained) – Total Care at Your Doorstep

Our mission is to bridge the healthcare gap by equipping patients with digital health tools, facilitating care coordination, and strengthening the connection between patients and their diasporan sponsors.
As we expand our reach, we are seeking compassionate and sales-oriented Nurse Case Managers to visit patients, writeup a patient assessment, provide hands-on assistance with downloading and using our healthcare app, and ensure seamless communication between patients and their sponsors.
Job Summary
The Nurse Case Manager will be responsible for conducting home visits to patients, providing basic healthcare guidance, facilitating app onboarding, and obtaining consent to share patient assessments with their diaspora sponsors. In addition to clinical expertise, this role requires strong sales and communication skills to educate patients and their families on the benefits of the platform and encourage active engagement.
Key Responsibilities
1. Patient Engagement & App Onboarding
• Conduct home/hospital/pharmacy/clinic visits to assist patients in setting up and using the healthcare app.
• Guide patients through downloading, registering, and navigating the digital platform.
• Establish a clear connection between the patient and their diasporan sponsor using the app.
• Ensure patients understand the benefits of the program and encourage the diaspora client to use the app versus sending the money through transfer services.
2. Health Assessment & Case Management
• Perform basic health assessments and document key findings.
• Collect and document patient history, current health status, and urgent care needs.
• Secure informed consent from patients to share assessment details with their diasporan sponsors.
• Coordinate follow-up care and referrals when necessary.
3. Sales & Relationship Management
• Educate patients and their families about the value of the service and address any concerns.
• Utilize persuasive communication techniques to ensure patient engagement.
• Act as a liaison between the patient and the diasporan sponsor, strengthening trust and participation.
• Provide feedback and reports to the program team on adoption rates and patient experiences.
4. Compliance & Data Security
• Adhere to patient confidentiality regulations and ensure data is securely transmitted.
• Follow ethical guidelines when obtaining patient consent and sharing health information.
• Maintain accurate and timely documentation of all interactions.
Qualifications & Skills Required
✅ Clinical Background:
• Registered Nurse (RN) or equivalent certification in Zimbabwe.
• Experience in community nursing, case management, or home healthcare preferred.
✅ Sales & Communication Skills:
• Experience in sales, patient education, or customer service is highly desirable.
• Ability to explain complex healthcare solutions in a simple, engaging manner.
• Strong interpersonal skills to build trust with patients and their families.
✅ Tech-Savvy & Adaptable:
• Comfortable using mobile applications and guiding patients through digital tools.
• Ability to troubleshoot basic tech issues during patient onboarding.
✅ Cultural Competency & Compassion:
• Understanding of Zimbabwe’s healthcare challenges and diaspora connections.
• A patient-centered approach, emphasizing dignity and respect.
✅ Organizational & Reporting Skills:
• Ability to document patient interactions accurately and submit timely reports.
• Strong time management skills for scheduling patient visits efficiently.
